The web site provides access to the proceedings and other information from the Third Joint Conference on Aging Aircraft, sponsored by the US Department of Defence, Federal Aviation Administration (FAA), and NASA, held in 1999. The site contains information on the sessions, participants and exhibitors. Specific contributions can be located using author browse, a drop-down menu of topic heading, or by browsing the "program at a glance". This site provides background details about the Accident Investigation Board of Finlan which investigates all major accidents regardless of their nature as well as all aviation, maritime and rail accidents and their incidents. Accident reports are written in Finnish with English summaries. An English version is prepared from important reports, for example, the MV Estonia (available in full-text). Accident Prevention


This is a monthly newsletter published by the Flight Safety Foundation, an independent body devoted to improving flight safety. Each issue of presents an in-depth examination of a single accident. This is based on an official accident investigation report from the US National Transportation Safety Board (NTSB), the UK Air Accidents Investigation Branch, the Transportation Safety Board of Canada or their equivalents in other countries. The report's essential facts and findings are edited to give a clear narrative of the accident and the subsequent investigation. Advanced Technology Aircraft Safety Survey Report


This web site provides access to an Australian Bureau of Air Safety Investigation (BASI) document titled: Advanced Technology Aircraft Safety Survey Report, Department of Transport and Regional Development, Bureau of Air safety Investigation, June 1998. The report contains the findings of a BASI survey which investigated pilot attitudes to advanced technology aircraft. Advancing Free Flight Through Human Factors


This is the full text of a report on a technical workshop which was held in 1995 and sponsored by the US FAA's Air Traffic Plans and Requirements Service and the Office of the Chief Scientific and Technical Advisor for Human Factors. The aim of the workshop was to begin to identify and solve human factors issues related to "free flight". Aging Aircraft Fleets : Structural and Other Subsystem Aspects


This web site provides access to a Research and Technology Organization (RTO) Educational Note, RTO-EN-015, dated March 2001. The document was produced by the RTO's Applied Vehicle Technology Panel (AVT) and the Consultant and Exchange Programme of RTO in support of a Lecture Series presented on 13-16 November 2000 in Sofia, Bulgaria. The United Kingdom Air Accidents Investigation Branch (AAIB) is part of the Department of Transport and is responsible for the investigation of civil aircraft accidents and serious incidents within the UK. This site provides access to the full text of the monthly Bulletins which are published in order to inform both industry and the public the facts as far as they are known, of accidents and serious incidents. They are available back to January 1996 and include details such as type of aircraft, number of people on board, details of any injuries sustained, history of the flight, and any recommendations.

The Air Accident Investigation Unit (AAIU) is part of the Department of Public Enterprise, and is responsible for the investigation of aircraft accidents and serious incidents within Ireland. The web site provides access to a variety of information resources. Several Formal accident and incident reports issued by the Air Accident Investigation Unit are available online. The site also provides access to Synoptic Reports which contain factual accounts of accidents and serious incidents, including recommendations, which have been determined up to the time of issue. There is also a chronological listing of recent reports. The site describes the procedures for the reporting of accidents and serious incidents, and contains the full text of a Statutory Instrument (S.I. No. 205 of 1997), The Air Navigation (Notification and Investigating of Accidents and Incidents) Regulations, 1997.

Airsafe.com is a personal web site, created by an airline safety analyst. It includes an analysis of serious air transport accidents, by airline and by aircraft type, since 1970. There are very brief summaries of the latest major accidents, and where available reference to further information sources on selected incidents. The site also provides access to the author's own Airsafe Journal, which is intended to highlight safey issues for, and on behalf of airline passengers.

This site acts as an online community for air traffic control professionals. The site provides a variety of facilities including a product centre which helps users identify information about products offered by members, industry news, email discussion lists, forthcoming events, the 'kiosk' which provides access to magazines, some of which are available free of charge (The ATC Expert), and a Library which offers searching for air traffic control-related documents.

The ATA is the trade organisation that represents the principal US airlines. Its role is to support its member carriers by promoting the air transport industry. Air Travel and Health


This web site provides access to the text of the 5th report prepared by the House of Commons, Science and Technology Committee, dated 15th November 2000. The report makes a series of recommendations aimed at safeguarding the health of airline passengers. It covers a number of areas on concern including: deep vein thrombosis (DVT); seating; ventilation; air quality; transmission of infection; filtration; noise; stress; in-flight medical emergencies; information for passengers, and complaints procedures. The report highlights a number of areas for further research including: the epidemiology of DVT; the demography of air travellers and the types and frequency of travel undertaken; real time monitoring of air quality and other aspects of the cabin environment; testing of blood oxygen levels across the whole spectrum of air travellers; the interaction between different aspects of the aircraft cabin environment; and the study of medical records of aircrew concerning any long-term effects from exposure to the aircraft cabin environment.

Aircraft Maintenance Safety Survey


This web site provides access to an Australian Transport Safety Bureau document titled: Aircraft Maintenance Safety Survey - Results, by Alan Hobbs and Ann Williamson, Department of Transport and Regional services, Australian Transport Safety Bureau, 2000. Aircraft Survivability: A Vulnerability Perspective Conference


The site contains full-text papers and presentations from a conference sponsored by the National Defense Industrial Association, dated October 21 - 23 1997, Naval Postgraduate School, Monterey, CA. Australian Society of Air Safety Investigators (ASASI)


The Australian Society of Air Safety Investigators (ASASI) was formed in 1978 after an inaugural meeting held in Melbourne. ASASI was formed to better serve and represent the views of Air Safety Investigators in Australia. Since then ASASI has grown to a membership of 141 and now hosts a biennial conference in conjunction with the New Zealand Society of Air Safety Investigators (NZSASI). ASASI is affiliated with ISASI. The site provides access to a newsletter, and past papers, including "Aviation Safety for the 21st Century in the Asia Pacific Region" presented by Brent Haywood, from the 1997 Asia Pacific Regional Seminar. The Australian Transport Safety Bureau (ATSB) is an independent Division-level unit within the Department of Transport and Regional Services that deals with the non-regulatory aspects of air, sea, rail and road safety. ATSB investigates accidents, incidents and safety deficiencies, and analyses safety data to prevent repeat occurrences and to minimise the effects of those that do eventuate. Automated Inspection of Aircraft


This final report (DOT/FAA/AR-97/69) was published by the Federal Aviation Administration in April 1998, and was written by C. J. Alberts et al. It summarises the development of a robotic system designed to assist aircraft inspectors by remotely deploying non-destructive (NDI) sensors and acquiring, processing, and storing inspection data [extracted from the author abstract]. Aviation Safety : Fourteenth Report

The Aviation Safety Data Exchange web site is sponsored by The Experimental Aircraft Association (EAA) with data assistance from the FAA to provide a source of important Safety Information for General Aviation, Amateur Built and Ultralight Aircraft. Many of the service difficulty reports, which are available upon request, are those which have been submitted by individual aircraft owners/builders and are not available at other locations. The site includes daily accident reports; relevant FAA Advisory Circulars; Airworthiness Directives; Service Difficulty Reports; safety tips and notes, and a listing of amateur-built aircraft kits.

Aviation Safety Network


Aviation Safety Network (ASN) is a privately maintained personal interest web site aimed at anyone with a professional interest in airline accidents and safety. The site provides access to a range of information resources. The Airliner Accident Database contains descriptions of over 5900 airliner write-offs in accidents since 1945. A list of aircraft types that have been included can be found in the Type Index. This provides brief aicraft specifications, plus a chronological list containing all aircraft accidents for that model in which the aircraft has been damaged beyond repair. ASN also provides links to a wealth of safety related information sources including: aircraft accident reports; Cockpit Voice Recorder (CVR) / Air Traffic Control (ATC) transcripts; features on a number of significant accidents and safety issues; accident statistics, publications; and an image gallery. The site also includes an Aircraft Accident Digest notification e-mail list and a moderated electronic discussion forum.

Aviation Safety Reporting System


This is a cooperative program established by the FAA, and administered by NASA. The ASRS collects, analyzes, and responds to voluntarily submitted aviation safety incident reports in order to lessen the likelihood of aviation accidents. ASRS data are used to: Identify deficiencies and discrepancies in the National Aviation System (NAS) so that these can be remedied by appropriate authorities; Support policy formulation and planning for, and improvements to, the NAS; Strengthen the foundation of aviation human factors safety research. The web site provides access to a range of information resources. There are twenty-seven ASRS Database Report Sets on topics of interest to the aviation community. Each Report Set consists of 50 ASRS Database records, preceeded by a note of introduction, caveats on use of ASRS data, and standard abbreviations and definitions used in ASRS Database records. ASRS produces two regular safety publications, CALLBACK and ASRS Directline. This is a virtual conference aimed at the aviation and aerospace industries. It was held between 5th and 16th March 2001 and the site will be available for the following 12 months. The conference uses the analogy of 'buildings' where the different activities take place. In order to view much of the content and to contribute, it is necessary to complete a free registration. Published on August 1 1998, this ACRM manual is a comprehensive implementation package including the crew resource management procedures, training of the instructor/evaluators, training of the crews, a standardized assessment of crew performance, and an ongoing implementation process. Effectiveness of Flight Attendants Attempting to Extinguish Fires in an Accessible Cargo Compartment


This is the full text of a technical note (DOT/FAA/AR-TN99/29) published in April 1999 which documents the results of a study carried out by the Fire Safety Section of the Federal Aviation Authority and sponsored by the US Department of Transportation (DoT). The project aim was to evaluate the ability of flight attendants to extinguish cargo fires in small Class B cargo compartments. Evaluation of Fire Test Methods for Aircraft Thermal Acoustical Insulation


This final report (DOT/FAA/AR-97/58) was published by the Federal Aviation Administration in September 1997, and was written by Patricia Cahill. It presents the results of laboratory round robin flammability testing performed on thermal acoustical insulation blankets and the films used as insulation coverings. This work was requested by the aircraft industry as a result of actual accidents involving flame propagation on the thermal acoustical blankets. Vertical flammability testing was performed as specified in Federal Aviation Regulation (FAR) 25.853, Appendix F. In addition, a cotton swab test developed by aircraft manufacturers was also evaluated [extracted from the Executive Summary]. This is a PDF file [23 pages, 82k] so Adobe Acrobat software will be required in order to read it.

This advisory circular (AC) presents guidelines for developing, implementing, reinforcing, and assessing Crew Resource Management (CRM) training programs for flight crewmembers and other personnel essential to flight safety. FAA Office of Accident Investigation


The Office of Accident Investigation (AAI) is the principal organisation within the FAA with respect to aircraft accident investigation and all activities related to the National Transportation Safety Board (NTSB). As such, AAI's mission is to investigate aviation accidents and incidents to detect unsafe conditions and trends and to coordinate the corrective action process. The site provides access to new NTSB Safety Recommendations. The Office of Accident Investigation is responsible for coordinating and tracking FAA responses to safety recommendations issued by the National Transportation Safety Board. System Indicators provides information on the current status and trends in the National Aviation System operations. Fire-Resistant Materials: Research overview


This final report (DOT/FAA/AR-97/99) was published by the Federal Aviation Administration (FAA) in December 1997, and was written by Richard E. Lyon. It provides an overview of the research being conducted by the FAA to develop fire safe cabin materials for commercial aircraft. The objective of the Fire-Resistant Materials program is to eliminate burning cabin materials as a cause of death in aircraft accidents. Long-term activities include the synthesis of new, thermally stable, low fuel value organic and inorganic polymer systems. The synthesis effort is supported by fundamental research to understand polymer combustion and fire resistance mechanisms using numerical and analytical modelling and the development of new characterization techniques [extracted from author abstract]. Flight Data Processing Techniques for Identifying Unusual Events


This web site provides access to a Virginia Polytechnic Institute and State University Department of Aerospace and Ocean Engineering PhD dissertation, by Iossif B. Mugtussids, dated 12 June 2000. The dissertation describes the use of a Bayesian classifier in order to analyse flight data. The objective is to distinguish between typical data points, that are present in the majority of flights, and unusual data points that can be only found in a few flights. By analyzing the recorded data it is hoped that precursors to hazardous behaviour can be identified and problems mitigated before they occur. Initial Development of an Exploding Aerosol Can Simulator


This technical note (DOT/FAA/AR-TN97/103) published in April 1998 reports on research performed by the Fire Safety Section of the Federal Aviation Administration and sponsored by the US Department of Transportation (DoT). The report describes the development of a simulator designed to replicate an exploding aerosol can in a consistent manner in order to investigate cargo compartment fires. The report includes a description of the simulator, and test reports of both open-air and confined-space tests. It is available in PDF format.

Interference Levels in Aircraft at Radio Frequencies Used by Portable Telephones


This web site provides access to a report produced by the Civil Aviation Authority (CAA) titled: Interference Levels in Aircraft at Radio Frequencies Used by Portable Telephones, Civil Aviation Authority, 2 May 2000. The report describes the results of interference tests performed on two types of civil transport aircraft at London Gatwick Airport on 15th February 2000. These tests confirm that transmissions made in the cabin from portable telephones can produce interference levels that exceeded demonstrated susceptibility levels for aircraft equipment approved against earlier standards. International Aircraft Fire and Cabin Safety Research Conference

International Aviation Safety Assessment (IASA)


The Federal Aviation Administration's IASA program focuses on a country's ability to meet international standards and recommended practices for aircraft operations and maintenance established by the International Civil Aviation Organization (ICAO). The IASA site contains definitions of the three rating criteria used for the countries at the time of the assessment, spreadsheets of countries and category in the Assessment Program (results are presented in either MS Excel or PDF formats) and an overview of the IASA programme. In addition a model civil aviation statute, model regulations, and model implementing standards are available.

The International Aviation Safety Association (IASA) was officially founded in New York on 4 Mar 99 and the site is intended to highlight and publicise global air safety issues. This web site contains the full text of the ICAO Safety Oversight Programme: Report of Assessment of United Kingdom, published by the UK Department of the Environment, Transport and the Regions, 10 December 1998. The ICAO assessment focused on three areas: personnel licensing; flight operations; and airworthiness of aircraft. International Civil Aviation Organization (ICAO)


The International Civil Aviation Organization (ICAO) was founded through the Convention on Inter-national Civil Aviation signed on 7 December 1944. It is the United Nations Specialized Agency responsible for establishing international standards, recommended practices and procedures covering the technical, economic and legal fields of international civil aviation operations. The ICAO web site provides access to a wealth of information resources. There is an informative "About" section, which describes the ICAO's foundation, aims, and structure, and contains the full text of the ICAO's Strategic Action Plan and Annual Report. The International Transportation Safety Association (ITSA) aims to improve transport safety in each member country by exchanging experience, promoting the practice of independent transport safety/accident investigations, to share information and to indentify common problems. Irish Aviation Authority


The Irish Aviation Authority (IAA) regulates the safety standards of Irish civil aviation and provides air traffic management in Irish controlled air space. The web site provides access to a range of information resources. National Transportation Safety Board (NTSB) : Public hearings


The NTSB, as part of its investigation into certain accidents, may hold a public hearing to record evidence presented by persons involved in the accident and by parties to the investigation. These hearings are held for the purpose of supplementing the facts discovered during the on-scene and subsequent follow-up investigation of the accident. Public hearings generally are held with regard to a major accident in which there is wide and sustained public interest, or significant safety issues. The web site provides on-line access to exhibit items and other detailed information from previous hearings. National Transportation Safety Board (NTSB) : Safety Recommendations


This is part of the National Transportation Safety Board web site. Safety Recommendations are issued as a result of the investigation of transportation accidents and other safety problems. They identify specific problems uncovered during a safety investigation and indicate how problems and deficiencies should be rectified. Letters containing the recommendations are directed to the organisation best able to act on the problem. The web site provides access to a most wanted list of transportation safety improvements. This includes recommendation numbers, status, and coverage of current and past items. The Most Wanted List is browseable by mode of transport. The site also provides statistical information on safety recommendations. These include acceptance rates (eg by mode of transportation and comparisons by year), as well as numbers of safety recommendations issued (by mode and by year). There is a searchable archive of Safety Recommendation Letters, and it is possible to subscribe to an NTSB mailing list in order to receive copies of Safety Recommendations.

National Transportation Safety Board (NTSB) Aviation Accident/Incident Database


The NTSB is an independent Federal agency charged by Congress with investigating every civil aviation accident in the United States and significant accidents in the other modes of transportation -- railroad, highway, marine and pipeline. The NTSB accident/incident database is the official repository of aviation accident data and causal factors. In the database, an event is classified as an accident or an incident. "Aircraft accident" means an occurrence associated with the operation of an aircraft which takes place between the time any person boards the aircraft with the intention of flight and until all such persons have disembarked, and in which any person suffers death or serious injury, or in which the aircraft receives substantial damage. The NTSB defines "incident" as an occurrence other than an accident, associated with the operation of an aircraft, which affects or could affect the safety of operations. The NTSB database contains only selected incident reports. The database is updated monthly.

The National Transportation Safety Board (NTSB) is an independent Federal agency charged by Congress with investigating every civil aviation accident in the United States and significant accidents in the other modes of transportation -- railroad, highway, marine and pipeline. The NTSB uses the information it gathers during accident investigations and the determination of probable cause to make safety recommendations to all elements of the transportation industry. While the recipient of a recommendation does not have to implement the proposed action, it does have to respond formally to the recommendation and specify what action is or is not being taken and why. The plan has been developed by the US National Science and Technology Council's Subcommittee on Transportation Research and Development, dated May 2000. Poulter Laboratory : Aircraft Safety Research


The Poulter Laboratory is engaged in studying the dynamic response of materials and structures to impacts, explosions, fatigue, corrosion, and fire. It has been engaged in several research programmes on aircraft safety and airworthiness. The web site describes several of these programmes including: fragment barriers to protect aircraft from a catastrophic engine failure; hardened luggage containers to protect aircraft from a terrorist bomb; evaluation of fracture development in aging aircraft from fatigue and corrosion; and testing and analyses of composite compressor blades subjected to bird strike loading. Regional Airlines Safety Study Project Report


This web site provides access to an Australian Bureau of Air Safety Investigation (BASI) document titled: Regional Airlines Safety Study Project Report, Department of Transport and Regional Services, Bureau of Air Safety Investigation, May 1999. Representative Values of Icing-Related Variables Aloft in Freezing Rain and Freezing Drizzle


This technical note (DOT/FAA/AR-TN95/119) was produced by the Federal Aviation Administration (FAA) in March 1996, and was written by Richard K. Jeck. Radiosonde and surface observations in freezing rain (ZR) and freezing drizzle (ZL), and a limited number of aircraft measurements in ZR, have been examined for information on the magnitude and altitude dependence of meteorological variables associated with aircraft icing in these conditions. The variables include temperature aloft, humidity (clouds), and windshear from the radiosondes; surface temperatures, ceiling heights, precipitation type and amount from the surface observations; and temperature, drop-size, rainwater concentration, and icing rate from the instrumented aircraft. These and other data are used here to arrive at tentative maximum and representative values of these variables [taken from author abstract]. Statistical Loads Data for Boeing 737-400 Aircraft in Commercial Operations


A final report (DOT/FAA/AR-98/28) produced by the Federal Aviation Administration (FAA) in August 1998 and written by John Rustenburg et al. The University of Dayton is supporting Federal Aviation Administration (FAA) research on the structural integrity requirements for the US commercial transport airplane fleet. The primary objective of this research is to support the FAA Airborne Data Monitoring Systems Research Program by developing new and improved methods and criteria for processing and presenting large commercial transport airplane flight and ground loads usage data. The statistical data presented include the initial recorded data previously reported in FAA report DOT/FAA/AR-95/21. The data include statistical information on accelerations, speeds, altitudes, flight duration and distance, gross weights, speed brake/spoiler cycles, thrust reverser usage, and gust velocities encountered [extracted from FAA abstract]. Universities Space Research Association


The Universities Space Research Association is a branch of the National Academy of Sciences. All member institutions have graduate programs in space sciences or aerospace engineering, and whilst the majority of the membership is American there are also members in Canada, England and Israel. Wake Vortex Bibliography


This annotated bibliography consists of abstracts of publications on aircraft wake vortices. Experimental and theoretical articles are included and consider the formation, structure, motion and decay of vortices and their effect on penetrating aircraft. The bibliography may be searched by author, title or abstract. It is produced by the Volpe National Transportation Systems Center of the U.S. Department of Transportation.
